Register with us as a New Patient

If you move into the Practice’ catchment area (see below) you are entitled to register at the Practice. Please note that you will be assigned to a GP and you should not ask to register with a specific GP, due to fair patient allocation amongst the GPs.

Please note that your medical records may take several weeks to arrive at the Practice, therefore we suggest you order a month’s supply of your regular medication from your existing practice before registering with us.

Those patients who are visiting relatives and need urgent and necessary treatment in our area, can register with us as a temporary resident for a maximum of 3 months, but if you need a supply of your regular medication, this can be sent to a Pharmacy of your choice from your current GP surgery.

We are happy to accept new patients who are ordinarily resident in the UK and living within our practice boundary.

We have the right to refuse to register a patient if they have a history of violence toward NHS staff within their medical record.

Step 1: Complete NHS Online Registration Form

Our online registration form will collect your details and record your preferences. This information may be added to your record and will allow us to begin to provide you with medical services.

Step 2: New Patient Health Questionnaire

Please deliver to the practice a completed new patient health questionnaire form.
